Person-centred planning is a holistic perspective and process in which all aspects of the person's life are to be considered including:
- Home
- School/work
- Leisure and recreation
- Cultural and spiritual
The person determines the lifestyle they want to live, what is meaningful day to day, and how they would like to be supported.
The person is the centre of all activity, all planning and decisions.
Support provided to the person will be guided by and reflect the vision of their individual choice.
Person-centred tools are the training and skills needed to seperate "what is important to someone" from "what is important for them." It is our committment/promise to those we serve:
